Diagnostics

The final diagnosis is made based on the medical history, physical exam, and results of laboratory and imaging tests.

Biopsy and pathology examination are performed prior patient’s preoperative therapy (chemo or radiation) or if surgery is not expected.

Check-up plan:

  • Physical examination, medical history, family history
  • CBC
  • Blood test for liver and kidney functuin (Comprehensive Metabolic Panel)
  • CA 19-9 antigen level in blood serum (sensitivity is 70–90%, specificity is 68–91%)
  • Chest, abdomen, pelvis CT scan
  • If CT is not possible:
    • Ultrasound of the abdominal organs, retroperitoneal space, small pelvis, cervical-supraclavicular zone
    • Chest X-ray
  • Esophagogastroduodenoscopy/EGD
  • ECG
  • Testing of BRCA1, BRCA2 and PALB2 genes by the NGS method. They influence the choice of chemotherapy regimen
  • MSI testing

Additionally:

  • Endoscopic ultrasound diagnostics (combination of endoscopy and intracavitary ultrasound methods)
  • Abdomen and pelvis MRI
  • PET-CT
  • Diagnostic laparoscopy
  • Chromogranin A level in blood serum (to differentiate adenocarcinoma and neuroendocrine tumor)
